She Picked Up Her Heart Medication. Then She Asked Me The Hardest Question.

I’m a pharmacist. Every single month, an elderly woman comes into my pharmacy to pick up her regular medications. Without fail, she always looks at me and asks, “How much today?” She doesn’t ask because she forgets the price; she asks because there are months when she simply cannot afford all of them. She has to manually choose which critical pills she is forced to skip.

Last month, she looked down at the counter and told me quietly, “Just the heart one. The rest can wait.” It broke my heart. She was actively choosing between her own vital organs, prioritizing her heart over everything else because she knew that if that one stops, none of the other medications matter anyway.

Seeing this, I decided I couldn’t just stand by. I started rounding down her totals. It’s never a massive amount—just a dollar here, two dollars there, whatever I can manage to slice off the bill.

She absolutely notices the change, but she never questions it out loud. Instead, we have built a beautiful, silent agreement. I adjust the final numbers on the screen, and she doesn’t ask why her total keeps getting smaller. It isn’t about charity; it’s about a pharmacist who simply refuses to watch a person choose which part of her body she can afford to keep alive this month.
