A 19-year-old woman had never menstruated, but she gave it little thought until her partner grew concerned about potential infertility after a year of trying to conceive.
Her initial gynecological exam showed typical external features: tall stature, fully developed breasts, and female genitalia. However, a physical evaluation revealed sparse pubic hair, a short vaginal vault, and no palpable cervix. A subsequent ultrasound confirmed the absence of a uterus and ovaries.
Blood tests showed high LH and FSH alongside male-range testosterone levels (650 ng/dL). A karyotype test revealed a 46,XY chromosome pattern, and an MRI located undescended testicles near the inguinal canals.
